Documents Required to Register Your Scholar

Here are examples of documents that Parents and Guardians could use depending on entering grade levels in order to register their scholars at Eagle Collegiate Academy:

  1. Parent/Guardian Photo Identification

  2. Student Birth Certificate

  3. Student Immunizations Record

  4. Two (2) documents that verify your residence such as:

  • Utility bill

  • Property Tax Statement

  • Escrow papers / Lease Agreement

  • Pay Stub with employer Name & your address

  • Correspondence from Government Agency                                                                                                                                   

   5. Most recent report card or transcripts and other relevant documents such as:

  • Individualized Education Program (IEP),

  • 504 Plan,

  • Individual Health Plan

  • Custody, Guardianship, and/or Other Legal Documentation
